Job Summary
As an Occupational Therapist (OT), you are key to helping us provide superior patient care. You will work to provide skilled Occupational Therapy services in accordance with the client’s plan of care to provide skilled occupational therapy service on an intermittent basis to clients in their homes in accordance with the physician's orders. We are looking for a team player who is eager to learn, passionate about providing quality care, and has a proven track record of compassionate care.
Obtain a medical history, particularly as it relates to the present condition. - Evaluate the client, the client’s significant other(s) and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the client, and what other agency and community services will be required. - Assure that the physician’s orders are appropriate and discuss necessary changes. - Supervise the Occupational Therapy Assistant (COTA), as needed. - Participate in agency activities, in‐services, and meetings, when appropriate. - Confirm, on a weekly basis, the scheduling of visits with the Director of Nursing, case manager, and scheduler. - Makes arrangements for outpatient services when procedures are required that cannot be given in the home. - Evaluate the effectiveness of occupational therapy intervention and update the client care plan as needed. - Prepare clinical/progress notes on the day of the visit which are incorporated into the chart within 72 hours. - Instruct clients in care and use of wheelchairs, braces, splints, and prosthetic and orthotic devices. - Teach, supervise and counsel the family and client in the total Occupational Therapy program and other related problems of the client at home. - Establish goals based on the client's current level of functioning and potential for improvement and perform re‐evaluations as indicated. Qualifications
Must be a graduate of an Occupational Therapy school approved by the Council of Medical Education of the A.M.A.
