Today, Larkin Street Youth Services is the largest provider of housing and services to homeless youth in the Bay Area. We serve 1,500 youth annually, operating or partnering to provide more than 500 emergency shelter, transitional, supportive, and subsidy-based housing beds at several unique sites across San Francisco. Under the direction and guidance of the Program Manager, you will be responsible for providing comprehensive, strength-based case management services to homeless and/or marginally housed transitional aged youth (ages 18-24). You will conduct a comprehensive evaluation of bio-psychosocial, emotional, educational, vocational and functional needs of young people; develop an individualized service plan in coordination with Larkin Street Youth Services continuum of care as well as leverages relevant community resources as needed. The Case Manager II leads collaborative service planning and must have a thorough understanding of the stages of change and development. The Case Manager II must be comfortable providing services to clients outside of normal office settings, including but not limited to emergency shelter environments, congregate living housing placements, scattered site housing, and single room occupancy (SRO) placements throughout the city of San Francisco. This is a full-time position, eligible for full benefits. Your responsibilities:

Rapid Re-Housing Programs
**Location:**Based out of the San Francisco office, the RRH Case Manager II completes home visits and case management sessions with their caseload at various locations in and out of the office. - This may include sites throughout the Bay Area generally within a 50 mile radius of San Francisco and accessible by public transportation. - **Hours:**9AM to 5PM OR 10AM to 6PM, depending on site needs. - **Working Days:**Tuesday through Saturday OR Sunday through Thursday. - **working schedule includes weekends. - Drop-In Center
Location: The Engagement and Community Center (ECC), 134 Golden Gate Ave, San Francisco, CA

You have some social services case management experience. - *BONUS*You have experience working in drop-in services, shelter, transitional, and/or permanent supportive housing programs. - You are most excited to serve Transitional Aged Youth (TAY), aged 18 to 25 of diverse backgrounds. - You are a self-starter and team player. Hourly Rate: $28 per hour
+$1.50 for bilingual incentive pay.
