What You Will Do:
Assist with personal hygiene tasks including bathing, grooming, dressing, and toileting to support client dignity and daily functioning (if required)
Support mobility and transfers such as assisting clients in and out of bed or chairs and providing standby support during ambulation
Perform light housekeeping duties such as sweeping, mopping, laundry, and cleaning to maintain a safe and sanitary living environment
Prepare and serve meals based on client preferences and dietary restrictions, including cleaning up after meals
Escort or accompany clients to appointments or errands using public transportation or walking, ensuring safety and comfort throughout
Monitor and document basic vital signs including blood pressure, temperature, and, if applicable, blood glucose levels, according to care plan requirements
Remind clients to take prescribed medications as directed in the care plan without administering or managing the medication
Observe and report changes in client condition including physical, behavioral, or environmental shifts to the assigned supervisor or care team
Maintain accurate records and documentation for each visit, including task completion, vitals, and any concerns noted
Build trust and rapport with clients by using empathetic, culturally competent, and trauma-informed approaches that promote client comfort and cooperation
Minimum Qualifications:
High school diploma or equivalent
Certification as a Home Health Aide (HHA), Nursing Assistant (CNA), or completion of a Personal Care Aide (PCA) training program
Must pass a background check and tuberculosis (TB) test
Must be available to work full-time, including some weekends and agency holidays, based on client needs
Have an in-depth knowledge of in home support services responsibilities, standards, and proper techniques, and proven ability to demonstrate proper techniques
Possess strong interpersonal communication skills with ability to communicate effectively/professionally with medical personnel
Access to reliable personal transportation required, including a DMV record that permits the driver to be insured under 4th Second’s automobile coverage
Pass Required Background Checks: Successfully complete Live Scan and other background checks as a prerequisite for hiring, ensuring compliance with safety and security protocols set forth by 4th Second
Preferred Qualifications:
Experience working with target populations such as individuals from diverse cultural, socioeconomic, and linguistic backgrounds, particularly those facing chronic illness, disabilities, or mental health conditions
Familiarity with Motivational Care Management models and how to apply them within a care plan
Willingness and ability to work with individuals facing chronic illness, active substance addiction, low self-compliance with health plans, and other complex social or health challenges
Experience working with Medi-Cal beneficiaries and knowledge of CalAIM Community Supports
Physical Qualifications:
Able to be physically active for duration of work shift, excluding breaks
Able to lift 40 lbs. - Able to push or pull individuals in wheelchair
Able to carry 25 lbs. up one flight of stairs
Able to walk up and down a minimum of one flight of stairs
Able to bend downward and forward at the leg, waist, and spine
Able to kneel on one or two legs
Able to stand for extended periods of time
Able to extend hands and arms in any direction
Work Environment:
This position operates in both residential care settings and the community. The role may involve exposure to unsanitary conditions, challenging behaviors, and various environmental factors.
